Intussusception during Infancy and Childhood


Abstract
This study is a factual review of 248 cases of intrussusception in infants and children (under 15 years of age) admitted to the Seoul National University Hospital from August, 1960 to July, 1975.
The some of the results are as follows; .
.1) Age, sex, and season. In our 248 cases, the youngest age was 6 days of age, and 192 cases were under one year of age (77.4%),
The pick incidence was from 4 to 8 months of age (55.2/). In sex distribution, male showed definitely higher incidence than in female with the ratio 2:1(167: 81).
Although the disease occurs the year around, April through July showed somewhat higher incidences.
2) In etiological analysis, 67.4% were thought to be idiopathic, inflammatory (including enteritis and swelling of the Peyer¢¥s patches) in 17.8%, following URI in 4.2%, neoplasm in 2.2% and anomaly (including Meekel¢¥s diverticulum, n1pbile cecum and ileocecal band) in 8.1%.
3) The most common anatomical type was ileocolic type (68. 6%). And there were ileocecal in 25.5%, enteric in 5.1%, and colocolic type in 0.8%, decreasing in order.
4) The cardinal symptoms and signs were mucoid bloody stool (currant jelly stool) in 88.1%, vomiting in 83.1%, irritability (pain) in 63.6%, palpable abdominal mass (sausage like) in 63.6% and fever (over 38¢¥C) & leucocytosis (over 10,000) in 47.5%.
5) In our cases, barium enema was tried in 74 cases(29. 7%), and of which 42 cases(57.1%) were successful.
6) At last 206 cases were resolved by operation manual reduction only in 86 cases, manual reduction with appendectomy in 88 cases, segmental resection with end-to-end. anastomosis in 12 cases and right hemicolectomy in 13 cases.
7) The complication rate was 5.9% (15/248) wound infection in 8 cases, incisional hernia in I case and adhesive ileus in 6 cases.
There was no difference in complications between manual reduction only and manual reduc-tion with appendectomy.8) The overall recurrent rate was 2.0% (5/248).
9) The mortality rate was 2.8% (7/248). during 1960-1969, it was 6.4% (6/94). during 1970-1975, it was 0.6% (1/154). The incidence of intussusception is slowly increasing now, but its mortality is markedly
decreasing now.
